From first call to first session
Book a free 15 minute call
Call either clinic or book online. Tell us you are an NDIS participant and whether your plan is plan managed or self managed. We will work out the right service and clinician with you.
We send a service agreement
It sets out the services, the fees and the NDIS support categories. You sign it and pass it to your plan manager. Self managed participants keep it for their own records.
Your first appointment
An assessment at Five Dock or Gladesville. We agree a programme and write your goals in the language the NDIA uses, so they carry across to your plan at review.
Ongoing sessions and reporting
Sessions run one to one. Ahead of a plan review we can write a progress report covering what has changed and what we would recommend next. Ask for it early.
Before your first appointment
- Your NDIS plan number.
- Your plan manager's contact details, if you are plan managed.
- Your support coordinator's details, if you have one.
- Any recent scans, surgical reports or letters from other clinicians.
- Clothes you can move in, and any equipment you normally train with.
